The Project ECHO Model: How the Model is Making a Difference in Rural and Undeserved Areas

Community Health Center, Inc. is the only Federally Qualified Health Center (FQHC) to operate its own Project ECHO® clinics. Weitzman ECHO clinics are specifically designed to meet the needs of safety net primary care providers and their communities. Participants will learn how ECHO can address knowledge gaps in clinical care and how ECHO builds a community of practice. Participants jwill also be able to describe the basic tenets of ECHO as compared to e-Learning.

Dr. Montalvo is the Associate Director of the Weitzman Institute, a division of Community Health Center Inc (CHCI). She is a nationally recognized leader with an extensive portfolio leading national initiatives in the primary care setting for both research and performance improvement. Her previous roles include: Northeast Director for the HRSA/Bureau of Primary Health Care National Health Disparities Collaborative, Chief Clinical Officer for the Community Health Care Association of New York, and Director of the NYSHealth Foundation Diabetes Campaign.

Dr. Montalvo currently serves at CHCI as the PI of the NIH Precision Medicine All of Us Research Program and a member of the Executive Committee. She is PI for Transforming Primary Care for LGBT People, a national initiative of NACHC funded by CDC. Dr. Montalvo serves on the advisory committee for the Hillman Foundation and the RWJF Future Nurse Scholars Program. Her Doctor of Philosophy is from Columbia University School of Nursing and she is an alum of the RWJF Executive Nurse Fellow Program and the Jonas Center Nurse Leader Scholar.
